If you submit the contact form, we receive the details you choose to share (name, phone, email, and your message) and use them only to respond to your enquiry and coordinate scheduling. Please do not include detailed medical information in the form. This website does not use advertising trackers.

Contact details are used solely to communicate with you about appointments and enquiries. We do not sell or share your information with third parties for marketing. Booking and payments are handled by third-party services (Cal.com and Stripe), each governed by its own privacy policy.
Health information & HIPAA
Protected health information shared in the course of treatment is handled in accordance with applicable law, including the Health Insurance Portability and Accountability Act (HIPAA). A Notice of Privacy Practices is provided to patients as part of intake. This website itself is not a secure channel for medical information. Please reserve clinical details for your telehealth visit or the secure patient portal of our booking partners.
Telehealth notice
Jubilee Time Health Care Services provides care via telehealth. Telehealth has benefits (access, convenience) and limitations (technology interruptions, and some situations that require in-person care). During intake we review whether telehealth is appropriate for your needs and what to do if an urgent situation arises between visits.
